Job summary

The role includes 15-20 hours per week with most hours billable or face-to-face intervention, plus 2–5 hours of weekly direct client contact depending on case complexity. Develop individualized service plans, coordinate with social agencies, provide counseling and advocacy, and document progress.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, or related field preferred (or equivalent).
• 1–2 years of experience in family support and case management.
• Strong knowledge of local resources and support systems.
• Strong communication and organizational skills with attention to case documentation.
• Proficient in Microsoft Office; experience with an EHR system is a plus.
• Bilingual applicants preferred but not required.
• Reliable transportation, valid driver's license, and current auto insurance required.

Responsibilities

• Develop individualized service plans with goals from referrals.
• Coordinate and implement community-based services with social agencies, hospitals, clinics, schools, and community resources.
• Provide counseling, support, advocacy, and service coordination with families.
• Report incidents and update supervisors and CYF caseworkers.
• Provide consultation to families and involved professionals.
• Assist with monthly meetings for participants and teams.
• Complete assessments, closings/aftercare, daily progress notes, and weekly billing documents.
