Job summary

Connections Health Solutions in King of Prussia, PA is seeking a part-time Family Support Specialist to provide family-centered, peer-informed support within our walk-in, observation and Crisis Stabilization Unit programs. You will draw on lived experience to build trust, connect families with resources, and help navigate the behavioral health system while collaborating with the clinical care team.

Qualifications

• High School Diploma or equivalent required.
• Lived experience as a caregiver, family member, or primary support person required.
• One year of experience in behavioral health or family support preferred.
• Ability to use lived experience to support, educate, advocate for, and connect with families.
• Strong written and oral communication; comfortable with computers and documentation systems.
• BLS must be obtained within seven days of hire.
• Must pass background checks: Child Abuse, FBI, PA State Police, and medical screenings.
• Vaccination policy compliance required; flu shot or masking exemption may apply.

Responsibilities

• Provide family-centered, peer-informed support to youth and caregivers within the CSU, walk-in, and observation programs.
• Build trust using lived experience to help families feel supported during crises.
• Collaborate with care teams to identify family needs and connect with resources and supports.
• Offer individualized referrals and concrete resources (housing, food access, transportation, mental health providers).
• Ensure timely and effective communication with individuals and caregivers regarding services.
• Support crisis and safety plan development with youth, families and the clinical team.
• Coordinate with guardians, foster families, caseworkers, and external agencies to align care plans.
• Provide educational handouts and psychoeducational materials tailored to diagnoses and family needs.
• Educate caregivers to advocate and navigate systems on behalf of the child or family.
