Job summary

Youth Advocate Programs, Inc. is seeking a Family Support Specialist to deliver community-based services and develop individualized plans. You will collaborate with social agencies, hospitals, schools and other resources to support families, and provide counseling, advocacy and coordination as part of a team.

The role requires strong organizational and communication skills, a valid driver’s license, and may involve bilingual Spanish capabilities. Training, supervision and weekly pay are included.

Qualifications

• Bachelor’s or higher in Mental Health or related field preferred.
• 1–2 years related experience in community-based family work.
• Knowledge of local community resources.
• Proficiency with data collection tools; EHR knowledge is a plus.
• Strong organizational and communication skills.
• Reliable transportation and a valid driver’s license.
• Bilingual (Spanish) is a plus.

Responsibilities

• Deliver quality community-based services per mission and core principles.
• Develop individualized service plans based on referrals.
• Coordinate with social agencies, hospitals, clinics, schools and resources.
• Provide counseling, support, advocacy and service coordination.
• Report incidents and update CYF caseworker and Director.
• Assist with monthly participant/team meetings and documentation.
