Job summary

Toward Maximum Independence is seeking a Direct Support Professional - Community Support Facilitator in San Diego. This role supports adults and children with intellectual and developmental disabilities in home and community settings, with a schedule offering both part-time and full-time hours.

The position involves assisting with daily living, developing and implementing plans, coordinating services, and promoting community participation.

Qualifications

• High School Diploma or GED is required.
• 1 year working in social services, education, or with the developmentally disabled population preferred.
• 3 years driving experience required.

Responsibilities

• Provide support to clients in their home and community including assistance with personal hygiene, if warranted by program.
• Complete required documentation including progress notes and medical appointment tracking according to established deadlines.
• Attend mandatory trainings and meetings including CPR, First Aid, Relias training, and staff meetings.
• Drive a personal vehicle to assist clients with their goals when needed.
