Grace Healthcare Services is seeking an On-Call Visit Nurse (RN) to provide compassionate hospice care in Morris/Sussex Counties. You will work in a dynamic team, with an RN Case Manager and IDT, to address urgent patient needs and ensure a seamless plan of care.

Responsibilities include on-call visits, documentation, collaboration with the care team, and maintaining high-quality standards while supporting families through challenging moments.

Formación

• Active RN license required.
• Experience in hospice, palliative care, or acute care settings preferred.
• Exceptional communication and strong organizational skills.
• Ability to thrive in a fast-paced, emotionally rewarding environment.

Responsabilidades

• Provide compassionate, on-call hospice nursing care to patients.
• Collaborate with the RN Case Manager and IDT to implement tailored care plans.
• Maintain accurate, thorough documentation of patient care.
• Participate as a vital member of the Interdisciplinary Team.
• Work under supervision of a Registered Nurse to assess patient and family needs.
• Ensure adherence to hospice standards and protocols during on-call visits.
