Grace Healthcare Services is seeking an On-Call Visit Nurse (RN) to provide compassionate hospice care in Morris/Sussex Counties. You will work in a dynamic team, with an RN Case Manager and IDT, to address urgent patient needs and ensure a seamless plan of care.

Responsibilities include on-call visits, documentation, collaboration with the care team, and maintaining high-quality standards while supporting families through challenging moments.

Monday - Friday 5pm - 8am and Friday- Monday 5pm - 8am
