Grace Healthcare Services is seeking a compassionate On-Call Visit Registered Nurse to serve patients in Sussex and Morris counties. This full-time role covers evenings and overnights on a 7on/7off schedule, collaborating with an RN Case Manager and the IDT to provide patient-focused hospice care.

The role emphasizes independent clinical judgment, thorough documentation, and a dedication to comfort, dignity, and support for patients and families in a fast-paced setting.

Qualifications

• Active and unrestricted RN license.
• Experience in hospice, palliative care, or acute care settings is highly valued.
• Exceptional communication and organizational skills.
• Willingness to work on-call in a fast-paced, emotionally rewarding environment.

Responsabilités

• Provide compassionate, on-call care to hospice patients addressing urgent needs.
• Collaborate with the RN Case Manager to implement tailored interventions for patients and families.
• Maintain accurate and thorough documentation to ensure high-quality care.
• Be an integral member of the Interdisciplinary Team (IDT) contributing to the care plan.
• Work under the supervision of a Registered Nurse to assess and meet patient and family needs.
• Deliver continuous, compassionate care in adherence with hospice standards.
