Job summary

Grace Healthcare Services in New Jersey is seeking an On Call Visit Registered Nurse to cover Ocean and Monmouth Counties. This Full-Time position runs Monday through Friday, 5pm–8am, with 7-on/7-off on-call shifts, and involves hospice patient care in home settings.

You will work with the RN Case Manager and the IDT to implement individualized plans of care, document carefully, and deliver compassionate, high-quality care under hospice standards.

Qualifications

• Active and unrestricted RN license required.
• Experience in hospice, palliative care or acute care preferred.
• Strong communication and organizational skills.
• Ability to thrive in a fast-paced, emotionally rewarding environment.
• Commitment to delivering high-quality patient care.

Responsibilities

• Provide compassionate, on-call hospice care to patients.
• Collaborate with RN Case Manager to implement interventions.
• Maintain accurate documentation of care.

• Assess and address multifaceted patient and family needs.
• Ensure adherence to hospice standards and protocols.
