Job summary

Grace Healthcare Services is seeking a Hospice Registered Nurse (RN) to join our Interdisciplinary Team in Morris County, NJ. The RN will assess and address the comprehensive needs of patients and families, initiate admission interventions, and maintain an ongoing plan of care throughout the patient journey.

Responsibilities include delivering skilled nursing care per care plans, collaborating with the IDT, educating families, and ensuring clear communication while advocating for patients'

Qualifications

• Active New Jersey or Multi-State RN license.
• Minimum of two years of nursing experience in acute-care settings (medical, surgical, or oncology).

Role Overview

As a Registered Nurse (RN) at Grace Healthcare Services, you will be an integral member of our Interdisciplinary Team (IDT), responsible for assessing and addressing the comprehensive needs of patients and their families. Valid New Jersey or Multi-State Registered Nurse (RN) license.

Minimum of two (2) years of nursing experience in acute-care settings such as medical, surgical, or oncology units.

Strong clinical practice with a focus on customer service, excellent communication abilities, and the capacity to work independently in a fast-paced environment.
