Job summary

Lifespan Therapy is seeking a Home Health Physical Therapist to deliver patient-centered care in geriatric in-home settings. You’ll perform comprehensive evaluations, design individualized care plans, and provide advanced therapy interventions in clients’ homes across the San Francisco area.

You will operate with autonomy, manage your schedule, and collaborate with a care team to ensure optimal outcomes, including documentation, referrals, and coordination of care.

Qualifications

• Valid California Physical Therapist license.
• CPR or BLS certification.
• Driver’s license and auto insurance.
• Physical + TB test (within the last year).
• Professional liability and business license (if applicable).

Responsibilities

• Provide Skilled PT Care: Conduct in-home evaluations and develop individualized plans of care for geriatric patients.
• Implement Therapeutic Interventions: Focus on functional strengthening, gait training, balance, and fall risk reduction to improve mobility.
• Manage Your Caseload: Schedule and conduct home visits based on availability and patient needs.
• Collaborate for Optimal Outcomes: Work with a supportive care team to ensure coordinated patient care.
