Job summary

Lifespan Therapy is seeking an experienced Home Health Physical Therapist to deliver patient-centered care in California. The role offers autonomy, independence, and an app-based workflow that handles credentials, scheduling, messaging, and invoicing.

You will work with a diverse geriatric patient population in home settings. Responsibilities include in-home evaluations, personalized treatment plans, gait/balance focus, and caseload management.

Qualifications

• Valid California Physical Therapist license required.
• CPR or BLS certification preferred.
• Driver's license and auto insurance may be needed for home visits.
• Physical + TB test within the last year.
• Professional liability and business license, if applicable.

Responsibilities

• Provide skilled in-home PT evaluations for geriatric patients.
• Develop individualized plans of care and implement therapeutic interventions.
• Manage caseload, schedule visits, and coordinate with the care team.
• Collaborate with supervisors to ensure safe, effective patient care.
