Job summary

WelbeHealth PACE seeks a Home Care Coordinator to perform in-home care assessments and set the framework for our home care team to help participants thrive. This role reports to the Home Care Manager and focuses on arranging, assessing, and overseeing personal care in participants' homes.

You will work a hybrid schedule with on-site IDT meetings and home assessments, coordinating with nursing, therapy, aides, social services, and physicians to deliver coordinated care.

Qualifications

• Healthcare licensure or equivalent, plus at least 3 years of professional experience.
• Minimum 3 years of case management or nursing in home/clinical settings with frail populations.
• Bachelor's degree preferred.

Responsibilities

• Coordinate incoming calls related to participants, physicians and services regarding orders, questions and referrals.
• Communicate with participants by phone and coordinate with nursing, therapy, aides, social services and physicians on schedule changes and test results.
• Monitor home care delivery and scheduling hours in collaboration with Home Care Services staff.
• Partner with Marketing to support enrollment of prospective participants into the program.
• Assist with staffing and scheduling activities and solicit manager input.
• Participate in end-of-life care coordination and support.
