Job summary

Habitat Health is seeking a Home Care Coordinator to ensure personal and clinical homecare needs are delivered, helping participants thrive in their homes and communities. You will assess needs, develop care plans, and coordinate home care services in collaboration with the IDT to provide safe, person-centered care in the home setting.

Role requires travel and a valid driver’s license; 80% of responsibilities occur in the community with required documentation in EHRs and adherence to policy.

Qualifications

• Minimum of two (2) years of clinical experience, preferably in home care, geriatrics, care coordination, primary care, or related healthcare setting.
• Knowledge of care planning, clinical assessments, and home care service delivery.
• Ability to conduct comprehensive participant assessments and identify changes in condition or care needs.
• Strong clinical judgment and critical thinking skills.
• Excellent interpersonal, communication, and collaboration skills.
• Ability to work within an interdisciplinary team environment.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate participant care needs, functional status, safety risks, and appropriateness of home care services.
• Perform home safety evaluations and provide recommendations to promote participant independence and reduce risk.
• Collaborate with participants, caregivers, and family members to identify care needs and service gaps.
• Coordinate home care services to align with the participant's individualized care plan and goals.
• Review weekly home care notes and documentation to identify service gaps and concerns needing follow-up.
