Job summary

WelbeHealth is seeking a Home Care Coordinator in California to coordinate in-home care assessments and oversee personal care for participants, reporting to the Home Care Manager. You will align with nursing, therapy, aides, and social services to ensure timely, quality care.

The role offers a hybrid schedule with onsite IDT meetings and home assessments, plus a competitive benefits package and compensation.

Qualifications

• Healthcare licensure or equivalent required; at least 3 years of professional experience.
• At least 3 years of case management or nursing experience with frail or elderly populations.
• Nursing knowledge and training to care for complex clinical conditions preferred.

Responsibilities

• Coordinate calls with participants, physicians, and services on orders, questions, and referrals.
• Notify participants by phone and coordinate with nursing, therapy, aides, social services, and physicians on changes and results.
• Track home care delivery and scheduling hours with the Home Care Services team.
• Support enrollment of prospective participants with the Marketing team.
• Assist with staffing, scheduling, and getting manager input.
• Participate in end-of-life care coordination and support.
