Job summary

Gentiva Hospice in the United States seeks an experienced RN Case Manager to lead in-home hospice care. You will assess patients, coordinate with the IDG, and ensure plans of care are followed with documentation and compassionate communication.

You will educate families, provide bereavement support, and collaborate with physicians, social workers, chaplains, and others to deliver patient-centered care across care transitions.

Qualifications

• RN license required in the state of employment.
• Minimum of one year RN experience in hospice, home health, or related areas.
• Hospice or palliative care experience preferred.
• CPR certification.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.
• Deliver and document skilled nursing care based on each patient’s Plan of Care—in home settings or wherever they call home.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Collaborate with an interdisciplinary team to build and refine personalized care plans.
• Provide education, support, and bereavement guidance to patients and loved ones.
• Teach caregivers how to care for their loved one safely and confidently.
• Communicate important updates in patient condition to the care team for timely interventions.
• Maintain accurate clinical documentation and follow infection control and compliance standards.
• Support quality improvement and scheduling initiatives to ensure compassionate and efficient care.
• Participate in on-call and patient support activities as applicable.
