Gentiva Health Services, Inc. is seeking a compassionate Palliative Care Nurse Practitioner to join our interdisciplinary team.

This role provides advanced palliative care to patients in their homes, guiding families through complex medical decisions and aligning treatment with patient goals. You will collaborate with the Palliative Medical Director, perform thorough assessments, develop individualized care plans, and manage treatment adjustments.

Qualifications

• MSN from an accredited program; state licensure required.
• Minimum two years as a Nurse Practitioner.
• Family or Adult Medicine specialization preferred.
• Experience with Part B billing is a plus.

Responsibilities

• Deliver comprehensive, home-based palliative care in partnership with the care team.
• Conduct in-depth patient assessments, including physical, psychosocial factors.
• Develop and adjust individualized care plans with patients and families.
• Prescribe medications and order tests as allowed by state law.
• Maintain accurate clinical documentation per professional standards.
• Coordinate care with physicians, caregivers, and other professionals.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals for additional care services when appropriate.
