Job summary

Del Corazon seeks a Hospice Nurse Practitioner (NP) in Santa Fe, NM to deliver compassionate in-home hospice care and partner with the Medical Director to ensure holistic patient support.

Qualified candidates hold a Master’s or Doctorate in Nursing, current NM NP licensure, APRN certification, and at least 1 year of hospice/palliative care experience, with strong assessment and communication skills.

Qualifications

• Master’s degree or Doctorate in Nursing and current NM NP licensure.
• 1 year of NP hospice/palliative care experience.
• APRN certification is required.

Responsibilities

• Conduct face-to-face visits with hospice patients in homes or facilities.
• Collaborate with Medical Director and other clinicians to ensure comprehensive care.
• Evaluate and manage symptoms, adjust care plans, and educate families.
• Document encounters accurately for regulatory compliance.
• Serve as a resource for clinical questions and family education about end-of-life care.
