Job summary

Del Corazon is seeking a Hospice Nurse Practitioner in Santa Fe, NM. Provide face-to-face visits, collaborate with Medical Director, and support patients and families.

You will assess and manage symptoms, document visits for compliance, and educate families. Requires NP licensure in NM, Master’s or Doctorate in Nursing, active APRN certification, and hospice experience.

We offer a flexible PRN schedule with compensation up to $200 per visit, plus mileage reimbursement for transportation.

Qualifications

• Master’s or Doctorate in Nursing with active NP licensure in NM.
• At least 1 year of hospice or palliative care NP experience.
• APRN certification currently active.
• Strong assessment and communication skills with empathy.
• Reliable transportation with mileage reimbursement.

Responsibilities

• Conduct face-to-face visits with hospice patients in homes or facilities to assess clinical needs.
• Collaborate with Medical Director and healthcare professionals to ensure optimal patient care.
• Evaluate and manage symptoms, adjust care plans as needed, and provide emotional support to patients and families.
• Document encounters accurately to ensure regulatory compliance.
• Educate families and caregivers about end-of-life process and available resources.
