Job summary

Del Corazon Hospice in Santa Fe, NM is seeking a PRN Hospice RN to deliver high-quality, patient-centered care at the end of life. You will develop and implement individualized care plans, manage pain and symptoms, and offer emotional support to patients and families.

Join a collaborative interdisciplinary team with flexible scheduling. A nursing degree and NM RN license are required, plus BCLS, TB testing, and background check.

Qualifications

• Graduate of an accredited school of nursing.
• Current NM RN licensure.
• 1 year acute, home health, community health or hospice experience in last 2 years.
• Current BCLS certification.
• TB testing and pre-employment drug screening; background check.
• Valid state driver’s license and reliable automobile; proof of insurance.

Responsibilities

• Assess patients' physical, emotional, and spiritual needs
• Provide direct patient care, including wound management, medication administration, and symptom management
• Collaborate with patients, families, and interdisciplinary team members to ensure coordinated care
• Educate patients and families on disease processes, treatment options, and self-care techniques
• Document patient care and communicate with team members, physicians, and other healthcare professionals
