Del Corazon is seeking a Nurse Practitioner to join its compassionate hospice team in Los Alamos, NM. The NP will conduct face-to-face visits with patients in homes or facilities, collaborate closely with the Medical Director, and support symptom management and care planning.

The role requires an NP with Master’s or Doctorate in Nursing and NM licensure, with at least 1 year of hospice or palliative experience and APRN certification.

Qualifications

• Master's or Doctorate in Nursing required.
• 1 year of NP hospice or palliative care experience preferred.
• APRN certification required.
• Strong clinical assessment and communication skills.
• Reliable transportation with mileage reimbursement provided.

Responsabilités

• Conduct face-to-face visits with hospice patients in homes or facilities.
• Collaborate with Medical Director and other healthcare professionals to ensure continuity of care.
• Evaluate and manage symptoms; adjust care plans as needed and provide emotional support to patients and families.
• Document patient encounters accurately to ensure regulatory compliance.
• Educate families and caregivers about end-of-life processes and available resources.
