Job summary

Del Corazon in Los Alamos, NM seeks a Hospice Nurse Practitioner (NP) to perform face-to-face visits and collaborate with the hospice Medical Director to ensure comprehensive patient care. This role focuses on symptom management, education for families, and empathetic patient support.

The NP will document encounters for regulatory compliance, provide clinical guidance to the team, and maintain a compassionate, patient-centered approach in home or facility settings.

Qualifications

• Master’s degree or Doctorate in Nursing with current NP licensure in New Mexico.
• Minimum of 1 year of NP hospice/palliative care experience.
• Current APRN certification.

Responsibilities

• Conduct face-to-face visits with hospice patients in homes or facilities.

Join our compassionate hospice team at Del Corazon, as a Nurse Practitioner (NP) providing essential support and care to our patients and their families. We are seeking a skilled NP to conduct face-to-face visits and collaborate closely with our hospice Medical Director to ensure comprehensive patient care.

Key Responsibilities

• Conduct face-to-face visits with hospice patients in their homes or facilities to assess and support their clinical needs
• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity
• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families
• Document patient encounters accurately and timely to ensure regulatory compliance
• Serve as a resource for clinical questions and assist in educating families and caregivers about the end-of-life process and available resources

Qualifications

• Education: Master’s degree or Doctorate in Nursing, with current NP licensure in New Mexico
• Experience: Minimum of 1 year of experience as an NP in a hospice or palliative care setting
• Certification: Current Advanced Practice Registered Nurse (APRN) certification
• Skills: Strong clinical assessment and communication skills, with an empathetic approach to patient care
• Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement provided)
