Job summary

Del Corazon Hospice in Santa Fe, NM seeks a compassionate Registered Nurse (RN) to provide hospice visits across Santa Fe and surrounding communities. The role emphasizes symptom management, comfort, and collaboration with an interdisciplinary team to ensure dignified end-of-life care.

The RN will manage assessments, administer treatments, and document in the EMR. A valid NM RN license and at least 1 year of nursing experience are required; hospice/home health experience is preferred.

Qualifications

• At least 1 year of nursing experience (hospice/home health preferred)
• Strong assessment, communication, and independent decision-making skills
• Ability to work autonomously in the field with time management

Responsibilities

• Case Manage hospice patients and visits in Santa Fe and surrounding communities
• Participate in nurse on-call rotation; on-call stipend and after-hours pay provided
• Conduct patient assessments and manage pain/symptom control
• Administer medications and treatments per plan of care
• Educate and support patients, caregivers, and families
• Collaborate with IDT team
• Complete timely documentation in EMR
• Advocate for comfort-focused, patient-centered care
