Job summary

Del Corazon is seeking a Nurse Practitioner to join its compassionate hospice team in Los Alamos, NM. The NP will conduct face-to-face visits with patients in homes or facilities, collaborate closely with the Medical Director, and support symptom management and care planning.

The role requires an NP with Master’s or Doctorate in Nursing and NM licensure, with at least 1 year of hospice or palliative experience and APRN certification.

Hospice Nurse Practitioner (NP) – Los Alamos, NM

Join our compassionate hospice team at Del Corazon, as a Nurse Practitioner (NP) providing essential support and care to our patients and their families. We are seeking a skilled NP to conduct face-to-face visits and collaborate closely with our hospice Medical Director to ensure comprehensive patient care.

Key Responsibilities:

• Conduct face-to-face visits with hospice patients in their homes or facilities to assess and support their clinical needs

• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity

• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families

• Document patient encounters accurately and timely to ensure regulatory compliance

• Serve as a resource for clinical questions and assist in educating families and caregivers about the end-of-life process and available resources

Qualifications:

• Education: Master’s degree or Doctorate in Nursing, with current NP licensure in New Mexico

• Experience: Minimum of 1 year of experience as an NP in a hospice or palliative care setting

• Certification: Current Advanced Practice Registered Nurse (APRN) certification

• Skills: Strong clinical assessment and communication skills, with an empathetic approach to patient care

• Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement provided)
