Job summary

OneWell Health Care is seeking a dedicated Direct Support Professional (DSP) / Caregiver for field-based, part-time work in Tybee Island, GA. The role involves compassionate, person-centered care for individuals with IDD and autism in home and community settings, following ISP/IPP and care instructions.

Responsibilities include personal care, daily living support, documentation, and coordination with a care team.

Qualifications

• Must possess at least one active certification: CNA, HHA, PCA or PCT.
• BLS certification required for Georgia staff.
• TB screening/testing may be required per program/regulatory requirements.

Responsibilities

• Assist individuals with activities of daily living and personal care.
• Monitor health and safety, report changes to supervisors.
• Complete required documentation and EVV records.
• Provide transportation or additional support as needed.
