Job summary

OneWell Health Care is seeking a Direct Support Professional in Evans, GA to provide compassionate, person-centered care to individuals with IDD and Autism in home and community settings. You will assist with daily living activities, health monitoring, and behavioral support to promote independence and quality of life.

The DSP will work field-based with flexible scheduling, require certifications (CNA/HHA/PCA/PCT) and a high school diploma, and manage EVV documentation and transportation in line

Qualifications

• English proficiency required, both spoken and written.
• High school diploma or equivalent is required.
• CNA/HHA/PCA/PCT certifications are preferred or required depending on program.

Responsibilities

• Provide direct, compassionate care and support for individuals with IDD and Autism.
• Assist with activities of daily living and community participation.
• Monitor health and safety and report changes to supervisors.
• Complete required EVV documentation and notes.
• Follow individualized service plans and care instructions.
• Transport or provide additional support as needed.
