Job summary

OneWell Health Care is seeking a Direct Support Professional (DSP) to provide person-centered services to individuals with Intellectual and Developmental Disabilities in residential, community, and home-based settings across New Jersey.

The DSP will implement ISPs, assist with daily living activities, promote independence, and ensure health and safety while maintaining dignity and confidentiality. A flexible schedule and field-based work are involved.

Qualifications

• High School Diploma or GED required.
• EHR documentation experience preferred.
• Basic computer skills and ability to use smartphones/tablets.
• Microsoft Office proficiency required.
• Valid driver’s license and ability to operate agency vehicles.
• Knowledge of person-centered support practices.

Responsibilities

• Assist with ADLs, medication support, meal preparation, mobility, transportation, and independent living skills.
• Implement ISPs and behavior support plans.
• Document services and report health/behavior changes.
• Maintain confidentiality and HIPAA compliance.
