Qualifications

• High School Diploma or GED required.
• Valid driver’s license and reliable transportation.

• Ability to work independently and exercise sound judgment.

Responsibilities

• Provide one-on-one support to individuals in their homes and communities.

• Monitor health changes and report updates to supervisors.
• Document services accurately and comply with HIPAA and state regulations.
