Job summary

Theoria Medical seeks a full-time Advanced Practice Provider to deliver consistent, high-quality care in local skilled nursing facilities. You’ll enjoy flexible scheduling and autonomy supported by a collaborative clinical team and intuitive technology to focus on residents over time.

Qualifications

• Active U.S. license and board certification in nursing or physician assistant scope.
• Specialties include internal/family/emergency if applicable to SNF care.
• Proven ability to provide patient-centered care in skilled nursing facilities.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding and address chronic/acute issues with treatment plans.
• Prescribe medications and coordinate with facility staff as needed.
• Maintain on-site attendance for scheduled shifts and comply with policies.
