Job summary

Theoria Medical is seeking a full-time Dual-licensed Advanced Practice Provider to serve local skilled nursing facilities in Michigan and Ohio. You will deliver high-quality, patient-centered care with a focus on continuity and trusted relationships with residents and facility staff.

Qualifications

• Active U.S. license to practice in Michigan and Ohio.
• Board certification by ANCC, AANPCB, PNCB, AACN (APN) or NCCPA (PA).

Responsibilities

• Provide high‑quality, patient‑centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on‑site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Education
