Theoria Medical is seeking a part-time Advanced Practice Provider to serve local skilled nursing facilities within a mission-driven care community. You’ll build trusted, long-term relationships and deliver high-quality, patient-centered care with flexible scheduling.

Qualifications

• Active U.S. license to practice medicine.
• Board certification by ANCC, AANPCB, PNCB, AACN (NP) or NCCPA (PA).

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounds, prescribe medications and treatment plans, address acute issues.
• Communicate with facility staff via phone or secure messaging.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards and facility policies.
