Nutrition that Works, LLC seeks a consultant dietitian to support the long-term care community in Jesup, GA, at 10 hours per week. Flexible schedule with good pay and drive time pay available.

Responsibilities include nutrition assessments, documenting with the NCP in the EMR, collaborating on care plans, and completing MDS - Section K, TF calculations, and wound/weight-change recommendations.

Qualifications

• Registered Dietitian credentials (CDR-credentialed) required.
• Valid dietetic licensure in the state of practice.
• Customer-focused with a positive attitude.
• Excellent communication and interpersonal skills.
• Client-centered approach to care planning.

Responsabilités

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF as needed.
• Make recommendations for wounds and significant weight changes.
