Nutrition that Works, LLC is seeking a consultant dietitian to join our team for the long-term care community in Warm Springs, GA. This part-time role involves providing nutrition assessments and care planning for 12 hours per week.

Qualifications

• Credentialed by the CDR and licensed as a Dietitian.
• Must maintain current dietetics licensure and credentials.
• Excellent interpersonal and communication skills required.
• Client-centered approach and positive, collaborative mindset.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and significant weight changes.
