Job summary

National Church Residences in Stark County seeks a Transition Coordinator to help adults move from long-term care to home or community settings. This is a Monday–Friday, in-person role serving multiple counties in Northeast Ohio, with travel to surrounding areas and non-weekend hours.

You will collaborate with healthcare and housing professionals, coordinate services, and ensure respectful, person-centered transitions under supervision.

Qualifications

• Education: High school diploma or GED required.
• Experience: Minimum 1 year in social service or healthcare (STNA) delivery with adults, elderly and family populations; 2 years' experience preferred.
• Knowledge of supportive services and community resources in assigned areas/counties.

Responsibilities

• Serve as liaison between healthcare and housing professionals; maintain knowledge of service lines.
• Collaborate with case managers to arrange services aligned with consumer preferences.
• Assist individuals in locating housing and obtaining materials/supplies.
• Support consumers in accessing entitled benefits and resources.
• Provide exceptional customer service by addressing questions, concerns, and problems.
