Job summary

CareSource is seeking a Transitions Coordinator for Post Acute to oversee member level of care transitions to long-term care, respite, and HCBS waiver services. You will work with Care Coordinators, Medical Directors, and providers to ensure timely and compliant transitions.

Required RN/LSW licensure in Ohio with 3+ years in case management or managed care; BSN preferred; CCM preferred. Remote within Ohio, full-time position with competitive pay and benefits.

Qualifications

• Minimum 3 years experience in case management and/or managed care.
• Current RN, LSW, or LISW licensure in Ohio required; multi-state licensure preferred.
• Case Management Certification (CCM) preferred.

Responsibilities

• Coordinate level of care transitions for post-acute and HCBS waiver services.
• Collaborate with Care Coordinators, Waiver Service Coordinators, and Medical Directors.
• Review eligibility, enrollment history, and demographics during case reviews.
• Ensure timely NF level of care determinations and discharge planning.
• Maintain documentation in EMR per accreditation and compliance standards.
• Escalate care coordination needs to appropriate team members when necessary.
