Job summary

All Abilities, Inc., a Lifesteps subsidiary, is hiring a full-time Nursing Home Transition Service Coordinator to help participants transition from nursing facilities back into the community. The role includes coordinating housing, utilities, and funding options while working closely with nursing home staff and families.

The position requires travel up to 50% weekly within a 50-mile radius of Hazelton, PA, strong communication, and proficiency with MS Office. Competitive benefits apply.

Qualifications

• High School Diploma or GED required with ability to communicate effectively and provide customer service.
• Ability to organize multiple tasks and meet deadlines.
• Travel up to 50% per week within 50 miles of Hazelton, PA using personal vehicle; mileage reimbursed.

Responsibilities

• Assist participants in finding housing and utilities options.
• Coordinate with nursing home staff and family members as needed.
• Check on waiver or income funding; coordinate with providers as required.
• May conduct case management in Luzerne, Lackawanna, Carbon, Schuylkill, or Northumberland Counties.
