National Church Residences in Stark County seeks a Transition Coordinator to help adults move from long-term care to home or community settings. This is a Monday–Friday, in-person role serving multiple counties in Northeast Ohio, with travel to surrounding areas and non-weekend hours.

You will collaborate with healthcare and housing professionals, coordinate services, and ensure respectful, person-centered transitions under supervision.

Qualifications

• Education: High school diploma or GED required.
• Experience: Minimum 1 year in social service or healthcare (STNA) delivery with adults, elderly and family populations; 2 years' experience preferred.
• Knowledge of supportive services and community resources in assigned areas/counties.

Responsibilities

This is an in-person , Monday-Friday role for candidates located in Stark County , serving and traveling to the following counties: Stark, Summit, Coshocton, Wayne, Holmes, Columbiana, Cuyahoga (eastern side), Medina, Mahoning, and Portage.

This exciting Transition Coordinator position works on-the-ground in the Northeast Ohio area and surrounding counties serving adults who are in need of housing and additional services to support their goal for independence. If you are an experienced Community Health Worker or STNA who wants to be out in the community, meeting clients where they are, this might be an exciting role for you!

Purpose

As a Transition Coordinator on the ground in Northeast Ohio, you would work Monday through Friday (no weekends) playing a vital role in helping adults move from long-term care facilities to home or community-based settings. Following organizational policies, state/federal regulations, and accreditation requirements, this position ensures a smooth, person-centered transition for each consumer, under the general supervision of the Home Choice Manager.

Essential Functions

• Serve as a liaison between healthcare and housing professionals, including service coordinators, discharge planners, and community-based service providers. Maintain knowledge of all service lines: adult day health, personal care/private duty, service coordination, skilled care, and hospice.
• Collaborate closely with individual case managers to arrange services that align with each consumer's preferences and community resources, including physicians, pharmacies, and other essential providers.
• Assist individuals in locating housing and obtaining necessary materials and supplies (i.e. social security card, ID, completing applications, etc.)
• Support consumers in accessing entitled benefits and resources.
• Provide exceptional customer service by addressing questions, concerns, and problems, ensuring daily needs are met.

Non-Essential Functions

• Assist with the completion of consumer workbooks when needed.
• Support participants with employment options if interest is indicated.

• Ensure all client interactions uphold Client Rights, respect, and dignity. Report complaints, abuse, misappropriation of funds/property, or any corporate compliance issues immediately.
• Adhere to all National Church Residences policies, including Employee Information Guide (EIG), facility handbooks, drug-free workplace, safe work practices, and all federal, state, and local regulations.
• Perform other duties as assigned.

Qualifications

• Education: High school diploma or GED required.

Travel

This Northeast OH based role will travel frequently throughout the week to visit and serve clients in the following counties: Ashtabula, Columbiana, Cuyahoga (eastern side), Geauga, Mahoning, Portage, Trumball and other assigned locations. This role is paid for travel time and offers reimbursement for mileage.

Licensure

Valid driver's license and compliance with National Church Residences motor vehicle policy is required.
