Qualifications

Responsibilities

• Serve as a liaison between healthcare and housing professionals, including service coordinators and discharge planners.
• Assist individuals in locating housing and obtaining necessary materials and supplies (ID, social security card, applications).
• Support consumers in accessing entitled benefits and resources.
• Provide exceptional customer service by addressing questions, concerns, and problems, ensuring daily needs are met.
