Qualifications

• College degree (preferably in human services) and experience delivering community-based services.
• Valid driver's license and reliable insured vehicle; willingness to travel.
• Maintain confidentiality of sensitive information.
• Strong time management and ability to meet deadlines.
• Complies with State/Federal acts and agency policies.
• Effective communication in the language and communication style of participants and families.
• Experience with housing is preferred.

Responsibilities

• Assist in housing search, voucher and housing applications.
• Attend housing appointments with the participant for housing authorities.
• Set up apartments as preferred by participant.
• Maintain communication with MRC case manager and updates on participant.
• Attend transition meetings prior to participant discharge.
• Track and document direct services, housing applications, and related communications.
