Job summary

AmeriBest Home Care, LLC in Philadelphia is seeking a Field Supervisor to plan, coordinate, and document non-skilled supervisory visits across the county. You will ensure compliance with regulations and mentor field staff while overseeing PCA supervision.

Qualifications

• High School Diploma required or equivalent; CNA or HHA preferred.
• Associate degree or equivalent experience preferred.
• Minimum 1 year healthcare experience.
• Proficient with Microsoft Office Suite or related software.
• Valid driver's license and reliable vehicle required.

Responsibilities

• Assess new client needs on admission and develop care plan.
• Confirm schedule for care and follow up with clients.
• Perform in-person/virtual reassessments and notify Team Lead of changes.
• Observe PCAs for annual evaluations and coaching.
• Assist in closing care gaps and conduct unannounced visits.
• Communicate problems and prepare incident reports.
• Maintain positive relationships with clients and staff.
• Maintain daily HHAX schedule and complete visits on time.
• Participate in staff meetings and trainings.
• Perform other duties and special projects as requested.
