Job summary

AmeriBest Home Care is seeking a Field Supervisor to plan, coordinate, and document non-skilled supervisory visits for clients in Philadelphia and surrounding counties. You will supervise PCAs, perform field evaluations, and mentor field staff to ensure compliance with policies and regulations.

The role requires strong communication, organizational, and driving responsibilities, with a focus on delivering high-quality home care services and maintaining excellent client relationships.

Qualifications

• Must have high school diploma or equivalent.
• CNA or HHA certification preferred.
• At least 1 year of health care experience.
• Proficient with Microsoft Office Suite or related software.

Responsibilities

• Assess new client needs on admission and develop the initial care plan.
• Confirm care schedule and follow up to ensure quality care.
• Perform in-person/virtual client reassessments and notify supervisor of changes.
• Conduct field observations of PCAs for performance evaluations.
• Assist in closing care gaps and conduct unannounced home visits as needed.
• Communicate client issues to staff and prepare incident reports.
• Maintain positive relationships with clients and demonstrate excellent customer service.
• Update daily schedule in HHAX and complete visits promptly.
• Participate in staff meetings and in-service trainings.
• Assist with special projects as requested.

The Field Supervisor is responsible for planning, coordinating, and documenting non-skilled
supervisory visits for the Philadelphia and surrounding county areas of clients following Home
Care policies and procedures. Field Supervisors assure compliance with local, state, and
federal laws, Medicare regulations, and established company policies and procedures.
The field supervisor is responsible for the direct supervision of the personal care assistant (PCA).
The field supervisor performs field evaluations and serves as a mentor and support for field staff

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Assesses new client needs upon admission and develops the initial care plan.
2. Confirms schedule for care and follows up with clients to ensure that quality care is received.
3. Performs in-person/virtual client reassessments and notifies Team Lead of any changes in the client's condition that may necessitate changes to the established care plan.
4. Performs direct observation of PCAs for annual performance evaluation and provisional monitoring of newly hired staff.
5. Assists in closing client care gaps and conducts unannounced home visits as necessary.
6. Communicates client problems, concerns, changes, and complaints to appropriate staff, and prepares reports of incidents that could result in legal action.
7. Maintains and develops positive relationships with existing and prospective clients, demonstrating excellent customer service and setting an example for other staff.
8. Maintains daily schedule in HHAX and accurately completes visits in a timely manner.
9. Participates in staff meetings, agency sponsored in-services, and team meetings, as directed.
10. Performs other duties and assists with special projects as requested.

REQUIRED SKILLS AND ABILITIES

EDUCATION AND EXPERIENCE

• High School Diploma
• CNA or HHA certification preferred.
• Associate's degree or equivalent industry experience preferred At
• At least one (1) year of previous experience in health care.
• Proficient with Microsoft Office Suite or related software.
• Must have the use of an automobile in good working condition, a valid driver's license, and automobile insurance compliant with company policies.

PHYSICAL REQUIREMENTS

• Ability to move, transport, or position up to 50 pounds
• Ability to move or traverse about in offices and or client homes, including ascending and descending stairs.
• Ability to communicate effectively and clearly with others to exchange information.

TRAVEL REQUIREMENTS

A valid driver’s license and reliable personal vehicle are required for this position.
