Adaptive Home Health is seeking a Field Supervisor (PT) to lead and mentor clinicians in Denver, overseeing orientation, competency checks, and field-based practice. You’ll balance patient care with teaching and contribute to AI-focused product improvements based on frontline experience.

The role emphasizes autonomy, leadership of new hires, and maintaining high clinical standards while contributing to program growth and patient access to home health services.

Qualifications

• Active, Colorado PT license
• Minimum 2 years of home health experience
• Clean disciplinary record — no adverse action in the last 12 months
• No licensure encumbrances within the last 5 years
• Current CPR/BLS certification
• Current Texas driver's license and comfort with field-based work
• Strong clinical assessment skills and OASIS knowledge
• Patient, encouraging communication style with ability to give constructive feedback
• Paraprofessionals (PTA, OTA, HHA) are not eligible

Responsibilities

• Precept and coach newly hired clinicians during patient visits, providing real-time clinical guidance and feedback
• Provide hands-on mentorship and support to new joiners, especially those with no home health experience
• Evaluate and sign off on new joiners' clinical competency through observation during patient visits
• Assist with skills lab training sessions and onboarding activities
• Administer and support end-of-orientation assessments for new hires
• Serve as a resource and primary contact for new joiners during orientation
• Maintain a reduced but active patient caseload to stay current while precepting and teaching
• Model best practices in OASIS documentation and adherence to CoP
• Identify concerns related to new joiner readiness or patient safety to Clinical Manager
• Participate in AI charting product improvement focus groups
