Residential Home Health and Hospice in Troy, MI is seeking a Clinical Field Staff Supervisor to lead and support our field clinicians. You will partner with field staff and office leaders to ensure safe, effective, and timely patient care while upholding our commitment to exceptional service.

The role requires a registered nurse with home health experience and some management background. You’ll oversee staffing, monitor performance, address patient and family concerns, and collaborate with sales

Qualifications

• Must be a registered nurse with home health experience.
• Minimum of two years RN experience in home care or similar setting.
• One year of management experience preferred.

Responsabilités

• Provide supervision and direction for field staff and act as liaison between field and office staff.
• Direct management of the POD's Clinical Field Staff, assigning tasks and establishing clear measurable objectives.
• Develop and engage Clinical Field Staff by fostering a motivating work environment that recognizes performance.
• Monitor clinicians via HCHB reports and update Clinical Records related to communications, orders, re-hospitalizations, conflicts, etc.
• Evaluate and address customer concerns, including documentation and resolution.
• Communicate with sales personnel regarding customer issues affecting referral source relationships.
• Evaluate client episodes to determine discharge vs. recertification and initiate orders as appropriate.
• Review staff schedules to assure productivity and provide weekly productivity projections to the Clinical Manager.
