Job summary

Adaptive Home Health, LLC seeks a Field Supervisor (RN) to guide clinicians in real-world home health practice, validate competency, and model top-tier care. The role is salaried, field-based in Detroit with a branch in Southfield, MI, reporting to the Clinical Manager and Director of Clinical Education.

You will precept, coach, and mentor new clinicians, maintain a reduced patient caseload, and contribute to product feedback to improve our charting and scheduling tools.

Qualifications

• Active, unrestricted Michigan RN license.
• Minimum 2 years of home health experience.
• Clean disciplinary record within the last 12 months.
• No licensure encumbrances within the last 5 years.
• Current CPR/BLS certification.
• Current Michigan driver’s license and comfort with field-based work.
• Strong clinical assessment skills and OASIS knowledge.
• Ability to give constructive feedback.
• Paraprofessionals (LPN) are not eligible.

Responsibilities

• Time allocation: 75% field work; 15% education; 10% projects.
• Precept and coach newly hired clinicians during patient visits.
• Provide hands-on mentorship to new joiners.
• Evaluate and sign off on new joiners' clinical competency.
• Assist with skills lab training and onboarding activities.
• Administer and support end-of-orientation assessments.
• Serve as primary contact for new joiners during orientation.
• Maintain a reduced but active patient caseload while precepting.
• Maintain quality standards (<15% missed visit rate).
• Model best practices in OASIS documentation and COP adherence.
• Identify and elevate concerns to the Clinical Manager.
• Participate in Adaptive improvement focus groups.
