Qualifications

• Associates degree is required; bachelors degree preferred.
• Current RN license in good standing.
• Minimum 2 years of nursing experience.

Responsibilities

• Oversee delivery of high-quality patient care within assigned region or caseload.
• Provide leadership and clinical oversight to nursing staff and care teams.
• Coordinate care plans with patients, families, physicians and interdisciplinary teams.
• Develop and adjust treatment plans; set goals with patients and evaluate discharge potential.
• Conduct supervisory visits and mandated assessments per policy and regulations.
• Serve as liaison between the agency, physicians, hospitals and external providers.
• Provide staff orientation, competency verification and in-field supervision.
