Job summary

Flatirons FlexCare for Seniors is seeking a compassionate Registered Nurse (RN) to provide clinical oversight and support for IHSS clients in their homes across the Denver Metro Area and Northern Colorado. This field-based role involves home visits, assessments, care plan development, staff supervision, and collaboration with families and case managers to ensure safe, compliant, and person-centered care.

Qualifications

• Active and unrestricted Colorado RN license.
• Valid driver's license and reliable vehicle.
• Willingness to travel throughout the Denver Metro Area and Northern Colorado.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage a flexible schedule.
• Comfortable providing care and conducting assessments in home environments.

Responsibilities

• Conduct client admission and initial assessments in the client’s home.
• Develop individualized care plans based on client needs and goals.
• Complete IHSS oversight and supervisory visits.
• Monitor client progress and identify changes in condition or needs.
• Review and update care plans as client needs evolve.
• Provide ongoing clinical oversight of IHSS attendants and direct-care staff.
• Train attendants on care plans, safety practices, and procedures.
• Communicate with clients, families, caregivers, case managers, and staff regarding care needs and changes.
• Travel regularly throughout the Denver Metro Area and Northern Colorado for home visits.
