Home Instead in Fort Collins, CO is seeking a dedicated Home Health Registered Nurse (RN) to oversee clinical care in the homes of seniors. The role includes client assessments, care plan development, caregiver training, and ensuring compliance with Colorado Class A home care regulations.

Qualifications

• Active Colorado RN license (in good standing).
• At least 1 year of experience in home health or geriatric care preferred.
• Knowledge of Colorado Class A home care regulations is a plus.
• Strong clinical assessment and documentation skills.
• Ability to train and support caregivers in a home setting.
• Compassionate, patient-centered approach to senior care.
• Reliable transportation and ability to travel to client homes in Fort Collins.

Responsibilities

• Conduct initial and ongoing client assessments to determine home care needs.
• Develop and update individualized care plans in accordance with state regulations and client needs.
• Provide medication setup, administration oversight, and training for caregivers and families.
• Perform wound care, oxygen therapy, injections, and other skilled nursing tasks within home care scope.
• Train and educate caregivers on proper techniques, infection control, and emergency procedures.
• Monitor client progress and collaborate with physicians, families, and interdisciplinary teams.
• Ensure compliance with state regulations, company policies, and best practices for home health care.
• Document assessments, care plans, and services provided in an electronic health record system.
• Be available for on-call nursing support as needed.
