Flatirons FlexCare for Seniors is seeking a compassionate RN to join our team on a contract/per-diem basis, providing clinical oversight and nursing support for home care clients across the Denver Metro area and Northern Colorado.
The Contract RN will conduct admissions assessments, develop care plans, supervise IHSS attendants, and train caregivers while traveling to client homes. Flexible hours and an independent, community-based role.

Flatirons FlexCare for Seniors is seeking a compassionate, organized, and experienced Registered Nurse (RN) to join our team on a contract/per-diem basis. This position provides clinical oversight and nursing support for home care clients throughout the Denver Metro area and Northern Colorado.
The Contract RN plays an essential role in client assessments, care planning, caregiver training, and ongoing supervision to ensure clients receive high-quality, person-centered care in their homes. This position offers a flexible schedule and is ideal for a nurse who enjoys independence, community-based care, and collaborating with caregivers, clients, and families.
Conduct initial client admission assessments in the home.
Develop individualized care plans based on client needs, goals, and physician recommendations.
Perform required IHSS (In-Home Support Services) supervision visits and documentation.
Provide clinical oversight of client progress and identify changing care needs.
Complete reassessments and update care plans as needed.
Train, educate, and provide ongoing support to home care attendants and caregivers.
Supervise IHSS attendants to ensure services are delivered safely and according to care plans.
Collaborate with clients, families, case managers, physicians, and agency staff.
Maintain accurate, timely, and compliant documentation.
Travel to client homes throughout the Denver Metro area and Northern Colorado.
Active and unrestricted Registered Nurse (RN) license in the State of Colorado.
Current CPR certification preferred.
Minimum of one year of nursing experience required; home care, community health, or case management experience preferred.
Knowledge of IHSS, Medicaid home care services, or long-term care preferred.
Strong assessment, communication, and organizational skills.
Ability to work independently and manage a flexible schedule.
Valid driver's license, reliable personal vehicle, and current auto insurance required.
Ability to travel regularly throughout the Denver Metro area and Northern Colorado.
Ability to travel frequently between client homes.
Ability to sit, stand, walk, bend, and lift up to 25 pounds as needed.
Ability to use computers, tablets, and electronic documentation systems.
