Job summary

Paradigm is seeking a Field Nurse Case Manager to cover the San Diego area in CA. This field/home-based role involves coordinating medical care for work-related injuries, with appointments at doctors’ offices and hospitals.

You will assess, plan, implement, and evaluate case management activities while collaborating with insurers, providers, attorneys, employers, and employees. Travel is expected within reasonable distances and the role emphasizes making a meaningful difference in patients'

Qualifications

• Active RN license in California.
• Experience in workers' comp case management preferred.
• CCM certification strongly preferred; within two years of hire.
• Strong organization, communication, and driving skills.

Responsibilities

• Manage medical case for work-related injuries from assessment to evaluation.
• Coordinate care with insurers, providers, attorneys, employers, and employees.
• Monitor progress and report results to stakeholders.
• Travel to appointments in the San Diego area; field-based work.
