Paradigm is seeking a Field Nurse Case Manager to cover the South Bay Area in CA and surrounding areas. This field, home-based role involves travel up to 2 hours one-way to doctors' offices and hospitals.

You will oversee medical case management for work-related injuries, coordinating care with insurers, providers, employers, and employees. Ideal candidates are licensed RNs with California credentials, experience in workers' comp case management, and certifications such as CCM/CRRN.

Formación

• Current, unencumbered Registered Nurse (RN) license in California.
• Experience in case management of workers' compensation cases preferred.
• Certifications (CCM, CRRN, CDMS, CRC) strongly preferred; within two years of hire.

Responsabilidades

• Manage medical case care for work-related injuries, including assessment, planning, coordination, implementation and evaluation.
• Collaborate with insurance carriers, medical providers, attorneys, employers and employees; monitor progress and report results.
• Maintain autonomy with minimal supervision and ensure clear patient education.
