Job summary

Paradigm is seeking a Field Nurse Case Manager to cover the South Bay Area in CA and surrounding areas. This home-based role involves travel to doctors' offices, hospitals, and other locations with up to 2 hours one-way travel per appointment.

Qualifications

Current, unencumbered RN license in California.
Experience in workers' comp case management preferred.
CCM certification strongly preferred; must obtain within two years of hire.
Strong organization, communication, and ability to work autonomously.
Valid driver’s license and good driving history.

Responsibilities

Manage medical case management for work-related injuries, including assessment, planning, coordination, implementation and evaluation.
Operate as home-based Field Case Manager with travel up to 2 hours one-way (4 hours round trip per appointment).
Coordinate with insurance carriers, medical providers, attorneys, employers, and employees; report progress to employer and carriers.
Collaborate with diverse stakeholders to ensure timely and effective patient care.
